  Personal information is being collected within the physical performance platform for the Canadian Armed Forces (CAF). The platform is managed by the Personnel Support Programs (PSP) of the Canadian Forces Morale and Welfare Services (CFMWS) under the authority of the National Defence Act.
  
  Your personal information will be used to create and manage your client profile, assess your individual fitness needs and limitations, administer fitness training and programs, track your health and progress, schedule appointments and provide feedback from PSP professionals. You may also sync your personal fitness device to the application to track your sleep, activity, and other fitness data. Personal information will be aggregated and used for data analysis, reporting, monitoring and program evaluation purposes.   
  
  The physical performance platform, Hexfit, is a cloud-based application provided by Hexfit Solutions Inc. Use of the platform is voluntary, unless otherwise directed by your chain of command. The personal information in your user account will be stored in the platform for a minimum of 2 years from the point that your service within the CAF has ceased.
  
  Personal information is protected, and is only used and disclosed by CFMWS in accordance with the Privacy Act, as described in personal information bank (PIB) CAF Fitness Data- CFMWS PPU 850 (under development). Under the Act, individuals have the right of access to, and correction of, their personal information, and the right to file a complaint with the Privacy Commissioner of Canada regarding the institution’s handling of personal information.
