The purpose of the Welcome form is to collect necessary information on families or individuals who need or may wish to receive specific services or information from Military Family Services (MFS).

Information will be kept confidential as per our privacy code policy. Privacy Policy

You have a few options for submitting the form:

  • Email it once completed
  • Print it and bring it with you on the day of the workshop
  • Fill out a copy in person, we’ll have forms available at the workshops check-in
